Large Review Finds Traditional Chinese Exercise Benefits Musculoskeletal Health
A major new umbrella review published in the British Journal of Sports Medicine suggests that traditional Chinese exercise may offer meaningful benefits for people with musculoskeletal disorders, especially when practiced consistently over time.
The study examined research on traditional Chinese exercise, a broad category that includes Taijiquan, Bāduànjǐn, Yìjīnjīng, Wǔqínxì, Qigong, Daoyin, and mixed or unspecified Chinese exercise programs. Rather than reviewing individual clinical trials one by one, the authors analyzed 92 meta-analyses that together included data from 1,744 randomized controlled trials. In total, the review produced 547 separate outcome associations.
Musculoskeletal disorders are among the most common causes of pain, disability, and reduced quality of life. They include problems such as knee osteoarthritis, chronic neck pain, low back pain, fibromyalgia, osteoporosis, and other conditions affecting the bones, joints, muscles, and connective tissues. Because many of these conditions are long-term, patients often need approaches that are safe, sustainable, low-cost, and adaptable to different levels of ability.
The review found that traditional Chinese exercise was associated with improvements in several important areas, including physical function, pain, stiffness, mental health, sleep quality, and bone mineral density. The authors reported high-certainty evidence for several outcomes, including improved physical function in knee osteoarthritis, improved mental health in patients with knee osteoarthritis and neck pain, improved sleep quality in fibromyalgia, reduced chronic musculoskeletal pain and stiffness with Taijiquan, and increased bone mineral density in people with osteopenia or osteoporosis.
Taijiquan made up the largest identified share of specific exercise interventions in the review, followed by Baduanjin, Yijinjing, Qigong, Wuqinxi, and Daoyin. Arthritis was the most commonly studied condition. The authors also examined exercise prescription factors and found that longer duration, greater frequency, and moderate-to-high weekly practice volume were associated with better clinical gains for knee osteoarthritis and osteoporosis.
This is an important point for the public. Traditional Chinese exercise is sometimes presented as though a few gentle movements should produce rapid results. The evidence suggests something more practical: these exercises appear most useful when practiced regularly and sustained over time. Like walking, strength training, stretching, or rehabilitation exercise, the benefit is not in the name of the method alone. It is in the repeated training effect.
The review is also a reminder that science is still discovering what traditional practices can and cannot do. Taijiquan and Qigong were developed within Chinese culture using concepts such as balance, breath, posture, intention, circulation, softness, and coordinated movement. Modern researchers study them through different language: function, pain scores, range of motion, sleep quality, depression scales, bone density, fall risk, and quality-of-life measures. These two ways of describing the body are not identical, but they can meet in useful ways.
At the same time, the study should not be read as proof that every Chinese exercise helps every musculoskeletal condition. An umbrella review is only as strong as the studies and meta-analyses it includes. Some research areas remain stronger than others, and exercise programs vary widely in style, intensity, teacher quality, duration, and patient population. The most responsible conclusion is not that traditional Chinese exercise is a cure, but that the evidence base is becoming broader, more organized, and more clinically useful.
For patients, the message is encouraging but realistic. Traditional Chinese exercises may be valuable additions to musculoskeletal care, especially for chronic pain, stiffness, functional limitation, and age-related decline. They are usually gentle, adaptable, and suitable for gradual progression. However, people with serious pain, unstable joints, fractures, neurological symptoms, or worsening disability should seek appropriate medical evaluation and guidance before beginning.
For clinicians, the review points toward a growing role for structured movement practices in long-term care. Traditional Chinese exercise may not replace conventional treatment, but it may help patients participate more actively in their own recovery.
